Admissions Process

Counties Served: Alamance, Cabarrus, Caswell, Chatham, Davidson, Davie, Forsyth, Franklin, Granville, Halifax, Mecklenburg, Orange, Rockingham, Person, Rowan, Stanly, Stokes, Union, Vance, Warren,  Burke, Catawba, Cleveland, Gaston, Iredell, Lincoln, Surry, and Yadkin


 The Villages of Hope Haven (Residential Aftercare for Men, Women, Families)

An applicant or referring agency completes and submits the Prospective Resident Form.  The submission of the Prospective Resident Form initiate contact with Hope Haven in an individual’s interest in his/her interest for admission to The Villages.

The applicant/referring agency must obtain the following required nine documents prior to an interview date being given:

  • Picture ID
  • Social Security card
  • Criminal Background Check
  • Proof of Physical Exam (within the last 60 days)
  • Prescriber Letter for Medication (on Hope Haven’s form)
  • Proof of recent TB Test Results
  • Verification of Homelessness Letter (if applicable)
  • Proof of Disability (Disability aware letter, copy of check, etc.)
  • Copy of Comprehensive Clinical Assessment

All documents should be faxed as soon as possible to Admissions Department’s fax line 833-328-5728

Once the applicant has turned in all required documents to Hope Haven, the prospective client will contacted for an initial interview.  If the applicant is approved for admission, then the applicant will be notified when a bed becomes available.


Serenity House – Concord (Resident Aftercare for Men)

An applicant or referring agency completes and submits the Prospective Resident Form.  The submission of the Prospective Resident Form initiate contact with Hope Haven in an individual’s interest in his/her interest for admission to Serenity House.

  • Picture ID
  • Social Security card
  • Criminal Background Check
  • Proof of Physical Exam (within the last 60 days)
  • Prescriber Letter for Medication (on Hope Haven’s form)
  • Proof of recent TB Test Results
  • Verification of Homelessness Letter
  • Proof of Disability (Disability aware letter, copy of check, etc.)

Charlotte Drive – Dilworth (Independent Living – Men Only)

An applicant or referring agency completes and submits the Prospective Resident Form.  The submission of the Prospective Resident Form initiate contact with Hope Haven in an individual’s interest in his/her interest for admission to Charlotte Drive.

  • Picture ID
  • Social Security card
  • Criminal Background Check
  • Proof of Physical Exam (within the last 60 days)
  • Prescriber Letter for Medication (on Hope Haven’s form)
  • Proof of recent TB Test Results
  • Verification of Homelessness Letter
  • Proof of Disability (Disability aware letter, copy of check, etc.)
  • Proof of Income (6 months)
  • Proof of Savings/Checking Account
  • 3 Letters of Recommendations
  • Proof of NA/AA involvement and sponsorship
  • Proof of County Residency

Arietta Black House – University (Independent Living – Women Only)

An applicant or referring agency completes and submits the Prospective Resident Form.  The submission of the Prospective Resident Form initiate contact with Hope Haven in an individual’s interest in his/her interest for admission to Arietta Black House.

  • Criminal Background Check
  • Proof of Physical Exam (within the last 60 days)
  • Prescriber Letter for Medication (on Hope Haven’s form)
  • Proof of recent TB Test Results
  • Verification of Homelessness Letter
  • Proof of Disability (Disability aware letter, copy of check, etc.)
  • Proof of Income (6 months)
  • Proof of Savings/Checking Account
  • 3 Letters of Recommendations
  • Proof of NA/AA involvement and sponsorship
  • Proof of County Residency
